Process Hazard Analysis
Structured studies that establish what can go wrong in a process, how likely it is and how severe it could be, so it is fixed on paper before it happens on the plant floor.

- Authority
- DISH, PESO and client standards
- Applies to
- Chemical, pharma, petrochemical, gas and other hazardous-process plants
- Our role
- Study lead and facilitator
What is included
- HAZOP and HAZAN studies
- Hazard identification and risk assessment (HIRA)
- Maximum credible loss scenario (MCLS)
- Job safety analysis (JSA)
- Event tree analysis
- Fault tree analysis
- Quantitative risk assessment (QRA)
Four steps, one point of contact
Collect data
We gather drawings, chemical data and operating procedures and define study nodes or tasks.
Workshop
A facilitated session with your process, maintenance and safety leads, node by node.
Analyse
Consequences and frequencies are assessed; QRA is modelled where the risk profile calls for it.
Report and track
A study report with a ranked action register you can track to closure.
What we will ask you for
Having these ready speeds things up. If something is missing, we help you put it together.
- P&IDs and process flow diagrams
- Material safety data sheets
- Plot plan and equipment layout
- Operating and maintenance procedures
- Time from your process, maintenance and safety leads for the workshops
Common questions
What is the difference between HAZOP and HIRA?
HAZOP examines a process line by line for deviations such as high pressure or no flow. HIRA is broader and looks at hazards across activities and areas. Many plants need both.
When do we need a quantitative risk assessment?
Usually for storage or handling of significant quantities of flammable or toxic materials, for new siting decisions, or when an authority or customer asks for it. We advise after reviewing your inventory.
How long does a HAZOP take?
It depends on the number of nodes and the complexity of the process. We scope it with you first and give a fixed plan before starting.
